Yes. This OM3 MTP-12 UPC trunk cable can be used with 40G QSFP+ SR4 multimode optics when the link is designed for MTP/MPO UPC interfaces and Type B polarity.
Can I use this OM3 cable for 100G QSFP28 SR4?
Yes, but OM3 usually supports a shorter reach than OM4 in 100G SR4 links. Check the transceiver datasheet and link loss budget before using OM3 for 100G.
